When scheduling an appointment, the receptionist will gather information to ensure enough time is allocated for your visit.

If you have a medical emergency, dial 911. If you have a question that requires immediate assistance, please call our 24 hour answering service at: (815)-741-6900. Your call will be returned by a physician.

Once Your Appointment is Scheduled

Co-Pay will be collected at the time of your appointment, or our physicians will not be able to see you.

Please arrive for your scheduled appointment on time. If you are unable to keep an appointment, please call us at least 24 hours in advance so we may use those times for other patients.
